Narrative

Shared Living is a residential option for people with disabilities (PwD) that provides them the chance to live in private family homes with dedicated caregivers. Each person plays an active role in planning their daily routine. They receive services tailored to their needs through LifeStream’s clinical team and experienced professionals.

Adult Family Care is a program that provides a residential option for people who cannot safely live alone. But they do not require the level of support of a more traditional group residence. AFC adults may move into the homes of host families. They may remain at home with their family members who provide daily care and the normal supports of everyday life in the privacy and comfort of a home space.
