Home health care is a Medicare benefit that can send a team of care professionals to your home. This team may include a nurse, therapist, aide, or other care professional. These professionals work with your physician to deliver a personal plan of care to your home. Home healthcare services can help you recover after a hospitalization or surgery or help you control the symptoms of a chronic illness such as diabetes, COPD, or heart failure. Hospice is a positive, empowering form of care designed to provide comfort and support to patients and their families when a life-limiting illness no longer responds to cure-oriented treatments. Patients who may benefit from hospice care include:
