Narrative

Individual Supports is a person centered program. The focus is to improve skills that will better a Member’s life either in their home or in the community. Members work one to one with staff on skills that will increase their independence. The supports are tailored to each Member’s own needs or in areas of personal growth.

The goal of the service is for the Member to increase independence at the skill being worked on. Much of these services are worked on after a person has either come home from work, or their Day Hab program. For most people given this service, they have supports weekly with an Individual Supports Specialist. Staff works with them anywhere from two hours to eight hours per week. Some examples of Individual Supports include:
