The Upham’s Home Health Care/Visiting Nurse program has been in operation since 1975 serving thousands of Boston residents. Upham’s Corner is the only Boston health center to have a home health care and visiting nurse program.

Our Home Health Care/Visiting Nurse program allows loved ones to return home sooner, remain in their homes longer and help prevent hospital re-admissions. The staff works with the patient and care team to develop an individualized treatment plan. The goal of the treatment plan is to have the patient recover from their illness or injury while remaining safe in the comfort of their own home.
