Narrative

All Care’s full continuum of services include:

  • Visiting Nurses - Through a highly collaborative approach, and based on the unique needs of each patient, the VNA team creates a personalized treatment plan to manage chronic conditions, maintain independence, and avoid unnecessary emergency room visits and hospitalizations.
  • Palliative Care Program - Palliative Care is specialized medical care focused on improving the quality of life for people with a serious illness. Those who struggle with pain and symptoms brought on by their advanced illness, but who may not be ready for or do not qualify for hospice, often benefit from palliative care.
  • Hospice - Hospice support empowers patients to live with dignity, grace and fulfillment. With a focus on caring, not curing, patients receive services wherever they call home, which is often in their home, a nursing home, hospital, or residential facility.
  • Private Care Services - Private duty nursing and care services.
  • Adult Foster Care Program - All Care’s Adult Foster Care (AFC) Program supports caregivers and the older adults and people with disabilities who rely on them for help with daily living activities. Caregivers receive a generous stipend, customized care plan, training, and support. Care recipients benefit from personalized care and the ability to remain at home.

All Care’s goal, in providing the full continuum of care, is to allow people the choice to remain at home for as long as possible. There is an individualized plan of care which provides peace of mind for them and their families.
