Narrative

With the Adult Family Care (AFC) program, qualifying adults have the opportunity to live in a family setting. This provides them the best possible quality of life while remaining part of the community. Our program provides a team of caring, talented professionals that provides health and care management services to its members. They carefully match each member to a home-like setting with a trained caregiver and provide on-going support and training. Any adult who has all of the following:

  • a chronic medical or psycho/social condition, or a developmental disability and is unable to live alone
  • need for help with at least one activity of daily living (ADLs), (eg, bathing, dressing, toileting, moving between positions, ambulating, or eating.
  • Enrolled in or eligible for MassHealth Standard, be enrolled in a Senior Care Option (SCO), or a One Care program.
  • Personal motivation to participate in the care plan.
